How to Apply

The Midwestern University Family Medicine OPTI program utilizes the ERAS (Electronic Residency Application Service) for our application process. Only those applying through ERAS will be considered. Please contact your Dean's Office for further information regarding this process.

Interviews will be scheduled following review of application materials. Application information is used by faculty to select individuals to interview whose goals and interests seem compatible with those of our program.

A completed application will consist of the following:
ERAS Application
Personal Statement
Curriculum Vitae
3 Letters of Recommendation (one from a Family Physician with who you have worked)
Medical School Transcripts
USMLE or COMLEX scores

Special Requirements:
Applicants must have a clear interest in Family Medicine.
Applicants are encouraged to have substantial and credible hands-on clinical experience (not observerships) of at least six months in the US within the last 2 years.
Applicants should have graduated from medical school within the last five years.

International Medical School Graduates:
Applicant must have passed COMLEX Step 1 & COMLEX Step 2, or USMLE Steps 1 and 2.

Interviews:
Earliest application dates will be determined by ERAS and is usually around August of each cycle. Deadlines for applications will be late December to Mid January of each cycle. Interview start date will be in the beginning of September of each cycle through mid January of the following year.
